Using Clinical Reasoning When Caring for Respiratory and Cardiovascular Conditions in the Older Adult

Discussion Purpose
The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to recognize typical and atypical cardiopulmonary presentations in older adults, synthesize diagnostic data to guide evidence-based management, and determine when independent management, collaboration, or referral is indicated. Emphasis is placed on clinical reasoning, patient safety, and appropriate escalation of care.
Post: Case Scenario
A 72-year-old man presents to the clinic with a four-week history of productive cough. He has a 10-year history of chronic obstructive pulmonary disease and a 45-year history of smoking two packs of cigarettes per day, having quit six years ago due to financial reasons. He reports chest soreness related to frequent coughing and has noticed dark-colored blood on tissue when coughing. Over-the-counter cough medications have not provided relief.
Vital Signs:
- Blood pressure 137/90 mmHg
- Heart rate 82 beats per minute
- Respiratory rate 22 breaths per minute
- Body mass index 23
Chief Complaint:
- “Persistent cough won’t go away with my normal cough medicine. I noticed blood on the tissue when I cough.â€
Fishbowl Discussion Structure
Students will be assigned to one discussion group (Group A, Group B, or Group C).
Each group will lead one clinical focus area and respond to the other two areas.

Group Assignments
Group A: Assessment and Differential Diagnosis
(Module 3 Learning Outcome 1)
Address the following:
- Identify key additional subjective and objective cardiopulmonary assessment data needed for this patient.
- Distinguish typical versus atypical presentations of respiratory and cardiovascular disease in older adults relevant to this case.
- Construct a prioritized differential diagnosis, explaining how the patient’s history influences your clinical reasoning.
Group B: Diagnostic Evaluation and Evidence-Based Management
(Module 3 Learning Outcome 2)
Address the following:
- Identify appropriate diagnostic studies (for example, imaging or pulmonary testing) to further evaluate this patient’s symptoms.
- Discuss how diagnostic findings would guide evidence-based cardiopulmonary management, including pharmacologic and non-pharmacologic interventions.
- Identify potential complications related to treatment decisions in older adults with chronic respiratory disease.
Group C: Collaboration, Referral, and Safety Considerations
(Module 3 Learning Outcome 3)
Address the following:
- Determine whether this patient can be managed independently by the nurse practitioner or requires collaboration or referral.
- Identify appropriate consultations and explain the rationale.
- Discuss patient safety considerations, including hemoptysis, medication risk, and monitoring for clinical deterioration.
