Discussion Purpose
The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to analyze complex medication regimens, apply evidence-based deprescribing principles, and communicate clinical reasoning collaboratively. Students will work in small groups to examine different aspects of medication management for an older adult and engage in professional dialogue across perspectives.

Fishbowl Discussion Structure
You will be assigned to one discussion group (Group A, Group B, or Group C).
Each group will be responsible for leading one section of the discussion and responding to the other two sections.
Posting Rules
- Each group submits ONE collective discussion post

Group Assignments
Group A: Deprescribing and Risk Reduction
(Lead Section)
Address the following:
- Identify one medication that may be appropriate to consider for deprescribing at this time.
- Justify your recommendation using the patient’s current symptoms, clinical findings, and evidence-based guidance for older adults.
- Discuss potential risks of continuing this medication versus benefits of deprescribing.
Group B: Medication Tapering and Symptom-Guided Management
(Lead Section)
Address the following:
- After completion of the prednisone taper, identify one medication that could reasonably be reduced or discontinued based on the patient’s reported symptom improvement.
- Describe how you would safely taper or discontinue this medication.
- Identify key clinical parameters that should be monitored during this process.
Group C: Chronic Disease Optimization and Patient-Centered Care
(Lead Section)
Address the following:
- Given the absence of heart failure exacerbation and adherence to a reduced sodium diet, identify any additional medication adjustments that could be considered.
- Explain your rationale for maintaining, adjusting, or continuing current therapy.
- Discuss how patient preferences and quality of life influence medication optimization in older adults.

Grading Note
This discussion is graded at the group level using the discussion rubric. Evaluation will focus on:
- Depth of clinical reasoning
- Use of evidence to support recommendations
- Quality of cross-section engagement
- Professional tone and clarity

- You should respond to at least two of your peers by extending, refuting/correcting, or adding additional nuance to their posts.
- All replies must be constructive and use literature where possible.
