Fishbowl Discussion: Hematologic and Immune Disorders, Frailty, and Clinical Reasoning in the Older Adult

After studying Module 2: Lecture Materials & Resources, discuss the following:

Discussion Purpose:

The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to construct differential diagnoses using hematologic and immune assessment findings, develop evidence-based treatment strategies, and apply culturally responsive and safety-focused management principles when caring for vulnerable older adults.

 

Post: Case Scenario

A 78-year-old woman presents for her first medical visit in three years, accompanied by a neighbor. The neighbor reports that the patient had been the sole caregiver for her sister, who died one week ago. At the funeral, the neighbor noticed that the patient appeared fatigued, confused, sad, and gaunt. The patient reports several months of physical and emotional strain while caring for her sister and endorses significant fatigue and sadness since her sister’s death.

The patient has never been married, lives alone, and has limited family support. She denies taking prescription medications and reports no known medical diagnoses.

Vital Signs:

  • Temperature 97.6°F
  • Heart rate 98 beats per minute
  • Respiratory rate 22 breaths per minute
  • Blood pressure 95/60 mmHg
  • Body mass index 21

Chief Complaint:

  • “Fatigue and sadness over the death of my older sister.”

 

Fishbowl Discussion Structure

Students will be assigned to one discussion group (Group A, Group B, or Group C).

Each group will lead one clinical focus area and respond to the other two areas.

 

Posting Rules

  • One group post only (collaborative submission)
  • Individual student posts are not permitted
  • Responses to other sections must be clearly labeled within the group post

 

Group Assignments

Group A: Assessment and Differential Diagnosis

(Module 2 Learning Outcome 1)

Address the following:

  • Identify key additional subjective and objective assessment data needed to evaluate this patient, with emphasis on hematologic and immune-related findings.
  • Construct a prioritized differential diagnosis, incorporating considerations such as anemia, immune dysfunction, malnutrition, frailty, depression, and systemic illness.
  • Explain how age-related changes influence the clinical presentation in older adults.

Group B: Diagnostic Evaluation

(Module 2 Learning Outcomes 1 and 2)

Address the following:

  • Identify laboratory studies that would assist in differentiating hematologic, immune, nutritional, and inflammatory conditions.
  • Discuss the role of screening tools relevant to this presentation (for example, nutritional, frailty, cognitive, or mood screening).
  • Explain how diagnostic findings guide clinical reasoning and next steps in management.

Group C: Management, Cultural Considerations, and Safety

(Module 2 Learning Outcomes 2 and 3)

Address the following:

  • Develop an evidence-based plan of care that integrates pharmacologic and non-pharmacologic interventions, with consideration of malnutrition, frailty, and immunosenescence.
  • Discuss patient education priorities, including nutrition, safety, psychosocial support, and follow-up care.
  • Address culturally responsive and safety-focused considerations, including social isolation, caregiver burden, and need for consultation or referral.

Required Cross-Section Engagement

In addition to leading the assigned section, each group must:

  • Respond to the other two sections by:
  • Expanding on differential reasoning
  • Identifying additional safety or cultural considerations
  • Offering alternative diagnostic or management perspectives
  • All responses must be integrated into one cohesive group post.

 

Submission Instructions:

  • Your initial post should be at least 200 words, formatted and cited in current APA style with support from at least 2 academic sources.
  • You should respond to at least two of your peers by extending, refuting/correcting, or adding additional nuance to their posts.
  • All replies must be constructive and use literature where possible.
  • Please post your initial response by 11:59 PM ET Thursday, and comment on the posts of two classmates by 11:59 PM ET Sunday.
  • You can expect feedback from the instructor within 48 to 72 hours from the Sunday due date.
