This activity will enhance their skills in assessing a patient’s physiological status and help them develop proficiency in documentation. For this assignment, you will be taking the patient vital signs.
Step 1: Preparation

1. Baseline Assessment
Assume a comfortable seated position. The nurse will measure the following vital signs:-Temperature (Normal: 36.5-37.5°C or 97.7-99.5°F)-Blood Pressure (Normal: 90/60 mmHg – 120/80 mmHg)-Heart Rate (Normal: 60-100 beats per minute)-Respiratory Rate (Normal: 12-20 breaths per minute)-Oxygen Saturation (SpO2) (Normal: 95-100%)Nurse: Record the vital signs in a structured format, including the age of the patient, date, time, and appropriate descriptors (e.g. mmHg or beats per minute). Identify and briefly discuss potential causes of abnormalities. Are these abnormalities expected or unexpected?
2. Pre- and Post-Medication Assessment
Patient: Select a patient being given antihypertensive medication or a medication known to affect vital signs (heart rate, oxygen saturation, temperature, etc.). Take a full set of vital signs before the patient takes the medication and repeat the vital signs one hour after the patient takes the medication. Nurse: Measure and record vital signs. Note any significant changes in vital signs after the medication was taken. Identify and briefly discuss potential causes of abnormalities. Are these abnormalities expected or unexpected?
