About Me
I am a nurse working as a case manager for a health plan, mostly work with Medicaid population.
I assist with coordination of care between PCP and specialist, chronic disease management, education, community resources
Assist with getting approval for procedures, DME, home health, transportation, finding providers that takes the plan.
I think the chapter that relates to what I do is Chapter 31: Chronic Care Policy: Medical Homes and Primary Care` or Chapter 30: The Politics and Policy of Disaster Response and Public Health Emergency Preparedness which ever is easier.
