Type of HIT Intervention: Telehealth Technology

Strategic Goal of HIT Intervention: To implement the Telehealth technologies as an intervention to improve healthcare outcomes in patients suffering from Chronic Disease

Impact and Findings:

Telehealth programs utilize health informatics, disease management, and other forms of technologies to not only, expand access to healthcare for communities which may be impacted by barriers, but also to improve the wellbeing of patients who may be affected by a varying type and severity of diseases. When used correctly, Telehealth can have a positive result on patients that suffer from chronic diseases. Telehealth has dramatically reduced death rates, reduced complications that occur from chronic diseases, and reduced hospital stays leading to the overall reduction in costs. Telehealth has allowed healthcare providers to communicate with their patients by telephone, webcam, and email, contributing to the increased efficiency and quality of care. The adoption of telehealth is on the rise, bringing with it the potential to dramatically reshape the manner in which healthcare is accessed and delivered (Burch, S., Gray, D., & Sharp, J. 2017). Telehealth can connect patients in different states. Doctors who have access can connect with patients safely and securely. Patients that live in rural areas would benefit significantly from Telehealth technology, by enabling them to receive the adequate care at the right time and most importantly the right place. The American Telemedicine Association has stated that more than 15 million Americans acquired remote restorative care in 2015. Patients can transmit vital details such as blood pressure, heart rates, and glucose results rapidly to their primary care doctor by utilizing new advancements. According to the American Hospital Association (2015), the process of having patients monitor their chronic illnesses at home has contributed to the reduction of hospital visits by as much as 50%.

Introduction

The purpose of the SMAC is to introduce, develop and evaluate Telehealth and Mobile health technologies to patients that suffer from chronic diseases. These technologies will aid in the improvement of the quality of care, delivery of care and overall health outcomes for veterans.

How telehealth will attempt to improve health outcomes for patients who suffer from chronic disease

Telehealth takes aim at providing care management for patients who are afflicted by chronic diseases including diabetes, congestive heart failure, hypertension, chronic obstructive pulmonary disease, and depression. This technology is also to aid U.S. Veterans with posttraumatic stress disorder by means of a Home Health Program. Additional functionality includes remote consultations for specialty physicians, as well as, patient medication adherence (J. Kvedar, M.J. Coye, & W. Everett, 2014). Telehealth will provide additional access to healthcare. Telehealth also increases the level of education of the patients. Patients can be more educated about their chronic disease and various treatments because of telehealth (J.Tan, Jones and Bartlett Publishers, 2010).

Health Management Information Systems

Telehealth involves the use of digital technology to share health information and provide healthcare services in an effort to improve accessibility and quality of care. By means of telehealth, patients have the ability to access educational material and support related to health via their computers, laptops, and other mobile electronics devices, thus providing the essential tools for them to self-manage their personal health. The increased use of mobile devices has occupied to light the term, mHealth, which is indicative of health information that is obtained by mobile technology. Telehealth creates a direct line of communication between a patient and a provider, a provider and other providers, and even, patients and other patients (J. Kvedar, M.J. Coye, & W. Everett, 2014).

Facility Information

The Veterans Hospital’s Mission is to provide quality healthcare to veterans using the abilities of all employees supported by the commitment to education and research. The Veterans Health Administration is America’s most extensive integrated health care system, providing care at over 1,000 healthcare facilities, including 170 medical centers and 1,063 outpatient sites of care of varying complexity (VHA outpatient clinics), serving 9 million enrolled Veterans each year (Administration, V. H. 2008). The NY Harbor Healthcare System is a part of that organization and is always engaged in improving the health outcomes of the men and women who have so proudly served our nation. They consider it a privilege to serve veterans’ health care needs in any way they can. The NY Harbor, Healthcare system, is available to veterans living in the five boroughs of New York City.

Health care statistics of veterans, the underserved and their chronic diseases:

Chronic illness means a disease that lasts three months or more that require ongoing medical attention to cure and affects a person’s daily life. Nowadays more than 70 million people in the USA ages 50 and older—four out of older adults—suffer from at least one chronic disease. (Hwang, Weller, Ireys, & Anderson, 2001) Chronic diseases are causing disability and death in New York State and throughout the United States. More than 40% of New York adults suffer from a chronic illness, and chronic diseases are responsible for 23% of all hospitalizations in New York State. Chronic diseases cause six out of ten deaths in New York State. Comparing with the general population, veterans served by the Veterans Health Administration suffer from three additional non-mental health diagnoses and one additional mental health diagnosis. (Veteran Health Administration, 2016) In the USA 35 percent (1.2 million) of Veteran health care users have 3 or more of the 29 chronic conditions. The health problem of chronic illness is extensive. People with chronic diseases often face some difficulties for basic tasks such as lifting objects or walking up steps, or daily life activities such as bathing, dressing, or eating. They have signed higher rates of hospitalization and made more emergency room (ER) visits. Their health care spending (shared among patients and payers) is higher than that for people without a chronic disease. (Hamine, et al., 2015).

Adhering to the VA Hospitals policies and code of ethics is a crucial element of the success of all patient health outcomes. It is imperative to maintain the Ethical Responsibility of the patients by giving them a sense of security during their healthcare delivery when using telehealth technologies. The VA hospital’s Essential Values include Integrity, which is to act with high moral principle and adhere to the highest professional standards when using the telehealth program. Maintain the trust and confidence of all of who are involved in the research for this technology. There is commitment to working diligently to serve the patients and fulfill individual and organizational responsibilities. Advocacy to be truly patient focused on identifying, fully considering, and appropriately advancing their interests. Respect to treat all those served and with whom we work with dignity and respect. Excellence will be maintained in striving for the highest quality and continuous improvement through this healthcare technology. The responsibilities of leaders are to be accountable for our actions, admit mistakes, and work hard in correcting them. This SMAC has demonstrated all the Essential Values pertaining to the technology used in the delivery of healthcare. Healthcare professionals have the moral obligation to make right decisions and provide the proper care that meets the patient needs. Attention should always be taken when planning and implementing any type of healthcare improvement to include the telehealth program in all areas concerning privacy and cybersecurity.

Managerial Abilities

· Initiative-Development of this managerial ability has be demonstrated by taking the initiative, as well as, acting on the opportunity to show the importance of an efficient telehealth program In taking the initiative to point the need to provide this service to patients that suffer from chronic diseases and sharing possible solutions to create a sustainable program has helped to develop my initiative ability.

· Use of Technology- By researching and learning the best way to implement the telehealth program, it has assisted me in developing this managerial ability. I attain knowledge on the technical and connectivity aspects of how telehealth works. The ability was also developed when I was evaluating digital and communication methods for enhancing healthcare delivery by using telehealth technologies.

· Developing others- This managerial ability was developed by assisting other students in the writing of their first SMAC. Team members were provided with the guidance and information that I have learned throughout the Master’s Program at ST. Josephs. They were encouraged to use and develop their ideas, so they felt included in the process of creating this SMAC.

· Planning- The development of this managerial ability has been demonstrated by my capability to determine the objective for increasing access to health care information and support, as well as, outline the necessary steps that would culminate in the effective implementation of a Telehealth System. As with any implementation of a new product or technology, there is a need to research the market needs, to generate ideas for the development phase, to collect data to support the effectiveness of the product and to set a reasonable timeline for the execution. The completion of such planning has enabled me to enhance this ability.

· Group Management- This managerial ability was developed through a collaboration with colleagues in an effort to successfully implement a Telehealth system intended to expand access to healthcare, effective communication, and the support of one another. While it is critical that we each held a degree of accountability, we demonstrated teamwork, particularly when faced with obstacles in our process.

· Persuasiveness- I was successful in developing this ability by applying data that was collected to support the team’s mission and, ultimately, achieve the audience’s buy-in to Telehealth technology. The success of the Telehealth is due in part to my ability to articulate the gains associated with the system, complete necessary training, and develop a firm, trustworthy rapport with our stakeholders.

· Using Technology- different types of systems that are used in healthcare are learned. How telehealth technology works and what the key factors to make this system better are explored. Being open to using new technology and searching for improvement developed my ability to use technology.

· Group Management- Having a group SMAC required managing others and ourselves. In teamwork, each person was responsible for others. We communicated and reminded each other what to do for next step during the researching progress. Follow up meetings were made to complete the works to finish it on time.

· Planning- Having a project that has a deadline forces us to have a long-term plan. To finish this project on time, I have made a list to follow up my plans to be successful. Priorities were determined to focus on the project. Assignment for the project was finished on time.

· Group Management- This managerial ability was developed by effectively working with my classmates to complete this assignment. Tasks were delegated, assigned, and completed using the SMAC outline and submitted on time.

· Developing Others- I have developed this managerial ability by sharing my systems work experience with my team. I’ve shared the technology telehealth uses to determine how this can positively affect the patients quality of care. My classmates are now aware of the technology functionality of telehealth.

· Written Communication- this managerial ability has been developed through the use of email correspondence within this group. I have developed a more mature writing style that includes clear ideas, punctuation, and time management. I’ve learned the importance of responding to emails on time and providing complete information
