Sensory, Dermatologic, and Musculoskeletal Assessment in the Older Adult

After studying Module 6: Lecture Materials & Resources, discuss the following:

Discussion Purpose

The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to conduct integrated geriatric assessments by examining how sensory changes, dermatologic health, and musculoskeletal function interact to influence safety, function, and quality of life in older adults. Emphasis is placed on clinical reasoning, patient-centered assessment, and appropriate diagnostic evaluation and referral.

The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to assess visual complaints in older adults, develop differential diagnoses, select appropriate diagnostic studies, and initiate evidence-based management and referral. Emphasis is placed on clinical reasoning, patient safety, and functional implications of vision loss.

 

Post: Case Scenario: Functional Decline Related to Sensory Changes

An older adult male presents with progressive difficulty performing activities of daily living related to declining vision. He reports difficulty recognizing people at a distance, increased sensitivity to glare, near falls related to visual misperception, and increasing difficulty reading and watching television. He continues to drive locally and asks whether his symptoms may be related to cataracts, noting that his wife previously had cataract surgery with symptom resolution.

Vital Signs:

  • Blood pressure 128/84 mmHg
  • Heart rate 82 beats per minute
  • Respiratory rate 18 breaths per minute
  • Body mass index 24

Chief Complaint:

  • “Decrease in my vision; glare is very bothersome.”

 

Fishbowl Discussion Structure

Students will be assigned to one discussion group (Group A, Group B, or Group C).

Each group will lead one assessment domain and respond to the other two domains.

Group Assignments

Group A: Sensory and Functional Assessment

(Primary Focus: Sensory Systems)

Address the following:

  • Identify key subjective and objective sensory assessment data relevant to this patient, with emphasis on visual changes.
  • Discuss how sensory impairment affects activities of daily living, mobility, and safety, including driving.
  • Explain how sensory findings guide clinical priorities and next steps in evaluation.

 

Group B: Dermatologic Assessment and Risk Considerations

(Primary Focus: Skin Integrity and Aging)

Address the following:

  • Identify dermatologic considerations relevant to this patient, including age-related skin changes and potential risks associated with sensory impairment.
  • Discuss how impaired vision may increase the risk of skin injury, delayed wound recognition, or environmental hazards.
  • Explain how dermatologic assessment contributes to comprehensive geriatric evaluation and patient safety.

 

Group C: Musculoskeletal Assessment and Fall Risk

(Primary Focus: Mobility and Function)

Address the following:

  • Discuss musculoskeletal factors relevant to this patient, including gait, balance, strength, and joint mobility.
  • Analyze how sensory impairment interacts with musculoskeletal changes to increase fall risk.
  • Identify assessment strategies or tools that support evaluation of mobility and fall prevention.
