After studying Module 4: Lecture Materials & Resources, discuss the following:
Discussion Purpose
The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to recognize subtle or atypical gastrointestinal and endocrine presentations in older adults, develop safe, age-appropriate treatment plans, and evaluate diagnostic testing and referral pathways using patient-centered clinical reasoning.
Post: Case Scenario
A 76-year-old woman presents to the primary care clinic accompanied by her daughter with complaints of fatigue, decreased appetite, early satiety, and intermittent abdominal discomfort over the past two months. The daughter reports that the patient has unintentionally lost approximately 10 pounds during this time and seems “slower†and more forgetful than usual. The patient denies overt abdominal pain, nausea, vomiting, or changes in bowel habits but reports feeling “just not right.â€
Her medical history includes hypertension and type 2 diabetes mellitus controlled with diet and metformin. She lives independently but has reduced her activity level recently. She reports no recent hospitalizations.
Vital Signs:
- Blood pressure 130/76 mmHg
- Heart rate 76 beats per minute
- Respiratory rate 18 breaths per minute
- Body mass index 21
Chief Complaint:
- “Fatigue and loss of appetite.â€
Fishbowl Discussion Structure
Students will be assigned to one discussion group (Group A, Group B, or Group C).
Each group will lead one clinical focus area and respond to the other two areas.
Posting Rules
- One group post only (collaborative submission)
- Individual student posts are not permitted
- Responses to other sections must be clearly labeled within the group post
Group Assignments
Group A: Assessment and Differential Diagnosis
Address the following:
- Identify key additional subjective and objective assessment data needed to evaluate this patient’s abdominal and endocrine symptoms.
- Discuss subtle or atypical gastrointestinal and endocrine presentations relevant to older adults that should be considered in this case.
- Construct a prioritized differential diagnosis, explaining how age-related changes influence clinical reasoning.
Group B: Diagnostic Evaluation and Clinical Reasoning
(Module Learning Outcomes 1 and 3)
Address the following:
- Identify appropriate laboratory tests, imaging, or diagnostic studies to evaluate potential gastrointestinal and endocrine etiologies.
- Explain how diagnostic findings would guide clinical decision-making, including ruling in or out serious pathology.
- Discuss when diagnostic findings would warrant collaboration or referral.
Group C: Management, Safety, and Patient-Centered Care
(Module Learning Outcomes 2 and 3)
Address the following:
- Develop a safe, age-appropriate treatment plan, incorporating pharmacologic and non-pharmacologic strategies.
- Discuss patient safety considerations, including medication risk, nutritional status, and functional impact.
- Address patient-centered factors such as health literacy, support systems, and follow-up planning.
Required Cross-Section Engagement
In addition to leading the assigned section, each group must:
- Respond to the other two sections by:
- Expanding on clinical reasoning
- Offering alternative diagnostic or management considerations
- Highlighting additional safety or referral issues
All responses must be integrated into one cohesive group post.
Submission Instructions:
- Your initial post should be at least 200 words, formatted and cited in current APA style with support from at least 2 academic sources.
- You should respond to at least two of your peers by extending, refuting/correcting, or adding additional nuance to their posts.
- All replies must be constructive and use literature where possible.
- Please post your initial response by 11:59 PM ET Thursday, and comment on the posts of two classmates by 11:59 PM ET Sunday.
- You can expect feedback from the instructor within 48 to 72 hours from the Sunday due date.
