Discussion Purpose
The purpose of this fishbowl discussion is to strengthen advanced practice nurses’ ability to diagnose conditions incorporating cognitive, neurologic, and functional findings, formulate evidence-based management strategies, and apply ethical, cultural, and safety principles when caring for older adults experiencing changes that affect independence and quality of life.
Post: Case Scenario
A 79-year-old man presents for his first visit to the clinic and reports a gradual decline in vision in both eyes over the past year. His prior medical records have not yet arrived. Since relocating from Florida one year ago, he has not had an eye examination and does not have an established ophthalmologist.
He reports increasing difficulty performing activities of daily living that require vision. He is unable to recognize people until they are close, is often startled by voices he cannot visually identify, and reports worsening difficulty reading and watching television. He notes glare sensitivity, several near-falls related to visual misperception, and continues to drive locally. He asks whether his symptoms could be related to cataracts, noting that his wife previously underwent cataract surgery with symptom resolution.
Vital Signs:
- Blood pressure 128/84 mmHg
- Heart rate 82 beats per minute
- Respiratory rate 18 breaths per minute
- Body mass index 24
Chief Complaint:
- “Decrease in my vision; glare is very bothersome.â€
Fishbowl Discussion Structure
Students will be assigned to one discussion group (Group A, Group B, or Group C).
Each group will lead one clinical focus area and respond to the other two areas.

Group Assignments
Group A: Assessment and Differential Diagnosis
(Module Learning Outcome 1)
Address the following:
- Identify key additional subjective data, including past medical history, social history, and relevant family history needed to evaluate this patient.
- Describe objective assessment components relevant to cognitive, neurologic, and functional status.
- Construct a prioritized differential diagnosis, explaining how cognitive and neurologic factors may influence the patient’s presentation and safety.
Group B: Diagnostic Evaluation and Clinical Reasoning
(Module Learning Outcomes 1 and 2)
Address the following:
- Identify appropriate diagnostic studies (including laboratory tests or specialized assessments) to evaluate potential neurologic or systemic contributors.
- Discuss when imaging or specialty diagnostic testing would be indicated.
- Explain how diagnostic findings guide clinical decision-making and management planning.
Group C: Management, Ethics, and Safety
(Module Learning Outcomes 2 and 3)
Address the following:
- Develop an evidence-based management plan, including initial interventions and follow-up care.
- Discuss potential risks or complications associated with treatment or delayed intervention.
- Address ethical, cultural, and safety considerations, including driving safety, fall risk, and patient autonomy.
- Identify whether consultation or referral is indicated and explain the rationale.

- Your initial post should be at least 200 words, formatted and cited in current APA style with support from at least 2 academic sources.
- You should respond to at least two of your peers by extending, refuting/correcting, or adding additional nuance to their posts.
- All replies must be constructive and use literature where possible.
- Please post your initial response by 11:59 PM ET Thursday, and comment on the posts of two classmates by 11:59 PM ET Sunday.
